He was predeceased by: his parents, Phillip Polk and Jennie Polk. He is survived by: his wife Ardith Sue Polk; his sons, Jonathan Polk, Matthew Polk and Daniel Polk; and his grandchildren, Ben Polk, Josh Polk, Sarah Polk, Ryan Polk, Gabe Polk, Dara Polk and Ethan Polk.
