He is survived by: his wife Doris Scott Hazelwood; his sons, Larry Wayne Hazelwood (Deborah Lee) and John Paul Hazelwood (Sherry); his grandchildren, Ethan Hazelwood and Emily Hazelwood; his great grandchildren, John Colton Hazelwood and Axton Frank Hazelwood; his stepchildren, Michael F. Scott (Maryam), Deborah Scott, Aubrey Bryan Scott (Barbara) and David Lee Scott Jr. (Susan); his stepgrandchildren, Emily, Julia Scott, Megan, Will, Scott Garnett, Katie Colgate and Allen Scott; his stepgreat-grandchildren, Owen Robert Zwanzig, Zeke Zwanzig and Charlotte Garnett; and his brothers, Clyde Hazelwood and Moyer Hazelwood.
