He is survived by: his parents, Sally Proctor (Ed Ostervich) of Braymer, MO. and Kenny Mayberry (Tammy Mayberry) of Brookfield, MO.; his children, Ash, Drake, Luke, Tommy Mayberry, McKenzie, Ashlin and Jayden Perry; his wife Bonnie Mayberry; his siblings, Cade Mayberry of Kansas City, MO. and Abby Simons (Minn) of Minneapolis; and his parents-in-law, John and Momma Rhu Griffin of Newton, Utah.. He is also survived by cousins, aunts, uncles.
