He is survived by: his wife Kelly Stephens of Washington, Utah; his sons, Dirk (Jo), Cam (Jaclyn), Chase (Season) and Tyson (Brittney); and his siblings, Kay Mortensen (Stephens) and Greg Stephens (Linda).
