He was predeceased by: his parents, Omer Alonzo and Eva Otterstrom Cook; his daughter Eva Jeanette Cook; and his brother Phillip Cook. He is survived by: his former wife Ernestina I. Jones; his daughters, Jennifer C and Jacqueline K; and his siblings, Eugene Cook, Don Cook, Merrill Cook, Verna Johnson and Clark Cook. He is also survived by nine grandchildren.
