He was predeceased by: his parents, Kenneth Curtis and Merle Curtis of Springville Utah; his daughter Christine; his siblings, Robert Curtis, Merrill Curtis and Jeniene; and his pets, Kalypso and MoJo.
He is survived by: his wife Carol Ann; his children, Kelly Alan Curtis (Jaune), Courtney Curtis, Katy Mikeal Curtis (Jeffrey) and Kimberly Curtis; and his sister Kathleen Orr (Richard).
