She was predeceased by: her parents, Leslie Ottobine Burnett and Jennie Myrteen Greenhalgh; her daughter Cynthia Lynn Smith; and her siblings, Ferron Ammon Greenhalgh, Larry Brent Burnett and Norma Kay Packard.
She is survived by: her children, Leslie Ann Broadhead (Val), Kim Lorraine Broadhead (Keith), Jesse Wayne Smith (Cori) of Santaquin and Danny Burnell Smith (Susan) of Santaquin; her siblings, Barbara Jean Bean and William George Burnett; and her brother-in-law Cleil Thayne Smith of Santaquin. She is also survived by 18 grandchildren, 39 great grandchildren, and 2 great-great grandchildren.
