He was predeceased by: his parents, Wayne Eugene and Nan Lorraine Burnett Smith; his sister Cindy Smith; his parents-in-law, Jack Craghead, Helen Craghead and Sally Atwood Craghead; and his siblings-in-law, Sandra McMeekin and Richard Craghead.
He is survived by: his wife Cori Ann Craghead; his children, Jake Smith (Brittney) of Santaquin, Nickolas Smith (Shiann) of Santaquin, Jordan Ellsworth (Kenny) of Mona and Jessa Smith (Sara) of Santaquin; his siblings, Leslie Broadhead (Val), Kim Broadhead (Keith) and Danny Smith (Susan); his uncles, Bill Burnett (Rada) and Cleil Smith (Mary); his aunt Barbra Bean; his siblings-in-law, Gary Craghead (Sheila), Bob Craghead (Martha), Jim Craghead (Dale) and Jackie McKinney (Jim); and his half-brother Terry Smith.
