He was predeceased by: his parents, Ken Underwood and Virginia Underwood; and his siblings, Vance, John, Jim, Carol Ainge, Tamara Henderson and Peggy.

He is survived by: his child Brain; his wife Vickie Diane Diamond of Eureka; his children, Coralee Smith (Wayne) of Nephi, Brian (Becky) of Eureka and Rhonda Draper (Mike) of Eureka; and his sister Fawn Fife of Payson. He is also survived by 9 grandchildren 18 great-grandchildren.
