She was predeceased by: her parents, Thomas Milton and LaRue York Campbell; her husband Paul Duane Perkins; and her children, Julie, Michael and Jeffrey.
She is survived by: her daughter Jan Holt (Tom); her grandchildren, Thomas Holt (Kristy), Scott Holt (Sarah), Sara Dalebout (Nate) and Tiffany Catona (Nick); her great grandchildren, Andur, Mara, Ben, Ian and Seth Holt; her grandchildren, Skye, Jackson Holt, Gabe, Brynn, Natalie, Aaron Dalebout, Kacie, Mischel and Davney Catona; and her brother Jerry Campbell (Marsha).
Viewing was held on Thursday, January 16th 2025 from 9:30 AM to 10:30 AM at the Church of Jesus Christ of Latter-Day Saints (9119 S 1300 E, Sandy, UT 84094). A funeral service was held on Thursday, January 16th 2025 at 11:00 AM at the same location. A burial was held on Thursday, January 16th 2025 at 2:00 PM at the Heber City Cemetery (680 N 550 E, Heber City, UT 84032).
In lieu of flowers Madge requested that contributions be made to Operation Smile, Children's Home Society of Minnesota, or American Cancer Research Center.
