He is survived by: his mother Betty Coates; his wife Angie Phillips; his children, Stephanie Phillips and Dwaine Elston Phillips (Sarah) of Jerry's; his grandchildren, Caden, Allison, Sean, Mariah and Blake; and his caregiver Jerry.
Viewing will be held on Saturday, January 25th 2025 from 12:00 PM to 12:45 PM at the LDS Church (868 Star Crest Dr W, Salt Lake City, UT 84116). A funeral service will be held on Saturday, January 25th 2025 at 1:00 PM at the LDS Chapel (868 Star Crest Dr W, Salt Lake City, UT 84116).
