He is survived by: his significant other Annette Hatch of Richfield; his children, Emily Heins of Monroe, John Hunt (Shelly) of Vernal, Kelly Hunt (Tami) of Glenwood and Angie Blomquist (Todd) of Monroe; and his sisters, Patricia Simonsen (David) of Richfield and MaryAnn Parker (Kirby) of Joseph.
