He was predeceased by: his parents, Floyd Arthur and Mildred Sophia Culmer Cook; and his granddaughter Shataya Mae Cook. He is survived by: his wife Londa While; and his children, Debbie (Lane), Kevin, Brett, Jalanne (Mike), Ryan (Rachel), Tony (Julie), Jeff (Stacey) and Katrina. He is also survived by his grandchildren, and great-grandchildren.
