He was predeceased by: his father Jeffrey J. Simmons Sr.; and his grandmother Ann Haycock. He is survived by: his child Development; his wife Kelsie Bohn Simmons; his son Jesse Byron Simmons; his mother Debi Simmons; his siblings, Jeff J. Simmons Jr., Miranda Simmons (Donnie Byrge) and Krystal Simmons; his grandfather Richard Haycock; and his parents-in-law, Randy Bohn (Tina), Roxanne Jensen (Kirk), Baylie Thomas (Bobby), Wade Bohn (Desteni) and Drew Bohn (Alyssa). He is also survived by 11 nephews, and seven nieces.
In Lieu of flowers, a bank account has been set up for donations at Utah Power Credit Union.
