He is survived by: his wife Cindy Cook; his siblings, Cherie Jolly, Ricky, Roger and Curtis Cook; his children, Jess Cook (Sara), Crystal Baraclough (Grant), Kaz Cook (Morgan) and Jaci Mohr (Colton); and his grandchildren, Kelby Campbell (Anthony), Isabell, Brylee, Maylee, Kole, Rhoby, Jaten, Presten, Taylen, Avaree, Donovan, Liam, Thoryan, Judah and Winter. He is also survived by children and grandchildren.
