He was predeceased by: his son Daniel J. Cook; his father Norris M. Cook; and his mother-in-law Vicki Van Ausdal. He is survived by: his mother Claudia Springer Cook of Nephi; his wife Kathy Cook of Payson; his children, Jerad S. Cook of Santaquin, Kimberlee A. Peterson (Corey) of Eagle Mountain and Katelyn L. Watters (Jalen) of Santaquin; his father-in-law Bud L. Van Ausdal of Santaquin; and his brothers-in-law, Darin Van Ausdal (Kim) of American Fork and Curtis J. Van Ausdal (Deena) of Highland. He is also survived by many nieces, nephews, aunts and uncles.

In lieu of flowers, the family asks to please consider donating to the American Cancer Society.
