He is survived by: his wife Annie L. Curtis Olsen of Santaquin, Utah; his children, Cody Jay Olsen (Lucinda) of Spanish Fork, Utah and Marci Cordova (Dino) of Provo, Utah; and his brother Shirl Olsen of Spanish Fork.
