He was predeceased by: his wife Linda Workman Van Dam; and his parents, Lois Erickson Van Dam and Jay Louis Van Dam. He is survived by: his daughter Paula Van Dam Thurston (Zack); his stepdaughter Jennifer Workman Bingman (Anson); and his sister Claudia Cook.
