He was predeceased by: his son Kurt Lewis Provost; and his great grandchild Rose Noelia Allred. He is survived by: his wife Lou Ann Provost of Santaquin; and his children, Jenice Allred (Kevin) of Vernal, Utah, Dan Ray Provost (Jeanine) of Salem, Utah, Wade F. Provost (Charline) and Amy Lou Provost of Pleasant Grove, Utah. He is also survived by 14 grandchildren; 27 great-grandchildren and 1 great great grandchild.
