He was predeceased by: his parents, Samuel Guy and Orlinda Nelson Ware; his children, Douglas, Leslie and Thomas; and his brothers, Samuel, Bruce, LaVar and Rue.
He is survived by: his wife Diane Thomas; and his children, Stuart (Brenda), Dennis (Tami) of Orangeville and Jennifer Johnson (Jeffrey) of Orem. He is also survived by 12 grandchildren, and 16 great-grandchildren.
