He is survived by: his wife Michelle Middlton; his children, Trisha Rodgers (Robert), Ricky Bates (Crystal), Nichole Kellerhals (Jeff) and Brieanna Healey (Brandon); his grandson Mitch Bates (Hannah); his brother Dave Bates (Kristen); and also, Karen and David Middleton. He is also survived by his children, but his grandchildren (14) and one great grandchild.
