He was predeceased by: his parents, Perry Elsworth and Marlen Whitehair (Broutout); his stepfather John Macon; and his mother-in-law Nancy Martines (Morris). He is survived by: his wife Cindy; his son Perry Stephen (Camille); his grandson Perry Isaac; his siblings, Karen Swartwood (Dean), Eric and Chris; his nieces, Amanda Kelsey (Michael), Kristina Brown (Adam) and Katherine Swartwood; his father-in-law Stephen Martines; his stepmother-in-law Denise Lindley; his siblings-in-law, Julie Graham (Martines) and Clayton; and his stepsisters, April Geis (Craig) and Angel Adkins (Chris). He is also survived by numerous aunts, uncles and cousins.
In lieu of flowers, the family requests donations be made to the family's second home, the Empress Theatre in Magna.
