He is survived by: his parents, Bruce Costa and Elaine Costa; his wife Rachael Lee Wallingford; his children, Keyara, Daxon and Thaddeus; and his brothers, Nathan (Melinda), Chris (Heather), Kevin (Staci) and Wes (Trina). He is also survived by aunts, uncles, cousins, nieces, and nephews.
