He was predeceased by: his parents, Amos Cook and Della Cook; his wife Betty Jo; his sons, Christopher and Cyle; and his brother Farrell.
He is survived by: his sons, Carter (Julia) and Curtis (Erica); and his siblings, Shirley Leslie (Dennis), Patsy Craythorne (Richard), Douglas Cook (Sue Ann) and Phillip Cook (Cathy). He is also survived by six grandchildren; five great-grandchildren.
