A wake was held on Friday, December 27th 2024 at 4:00 PM at the Carmelita Jack Residence (3200 4500 E, Fort Duchesne, UT 84026). A funeral service was held on Saturday, December 28th 2024 at 10:00 AM at the Ft. Duchesne Community Building. A burial was held on Saturday, December 28th 2024 at the Fort Duchesne Cemetery (6766 U. S. Hwy 40, Randlett, UT 84026).
