She was predeceased by: her parents, Curtis Butler and Mabel Butler. She is survived by: her husband Jay Evans; her sons, Greg Evans (Paula) of Eureka and Allen Evans (Inthuorn) of Lexington; her grandsons, Zane Evans (Brooklyn) of Payson, Utah, Curtis (Kiona) of Draper, Utah, Aydin and Raffee Evans of Lexington; her great grandchildren, Nora and Owen Evans; and her siblings, Randy, Charlie Butler and Barbara Nelson.
In lieu of flowers the family suggests donations be made in Sandy's name to the Eureka United Methodist Church, the American Red Cross, or Crossroads Urban Center.
