He is survived by: his parents, Barbara Garlick (Evanston) and Matthew Ellsworth; his children, Tyliegh, Gavin and Owen; his wife Sierra Stephenson; his stepmother Coco Ellsworth (West Valley); his siblings, Tyliegh (Clayton) Nielson (American Fork) and Owen Ellsworth (American Fork); his grandparents, Dale, Betty Garlick (Mona), Randy and Connie Ellsworth (Eagle Mountain); his nephew Liam Nielson (American Fork); and his brother Dexter Ellsworth. He is also survived by many aunts, uncles, and countless cousins.
