He was predeceased by: his parents, John Vaculik and Maria Rabl Vaculik; his sister Marianne Christine; and his brothers-in-law, Gary Docekal and Don Stringham.
He is survived by: his wife Martha of Draper, UT; his daughters, Julie Jeffs (Mark) of Draper, UT and Jennifer Maw (Tyler) of Hernando, MS; his granddaughters, Alexandria Levatino (Kaolin), Haley Jeffs, Mackenzie Jeffs (Kolten Smith) and Lindsay Jeffs; his great grandchild Emelia Levatino; and his siblings, Annelise Stringham and Johannes Vazulik (Jean).
In lieu of flowers a donation can be made to St. Jude Children's Research Hospital.
