She was predeceased by: her mother Freeda Perkins Guymon; her significant other John Calvin Blake; her daughter Freeda Ann; her father David Lavar Guymon; and her siblings, Leonard, Clark, Keith, Ronald and Lorraine.
She is survived by: her children, Jeffery Calvin (Jodilyn) of Blanding, Utah, Callie Dawn Winters (Scott) of Lehi, Utah, James Lynn Blake (Cynthia) of Syracuse, Utah, Marcia Kay Hawkins (Darel) of Salem, Utah and Kevin Dean Blake (Cindy) of Blanding, Utah. She is also survived by 39 grandchildren, 56 great-grandchildren and 3 great-great-grandchildren.
