He was predeceased by: his parents, Albert Booth and Vesta Booth (Dixon); and his brother Gordon. He is survived by: his wife Carrie Hendricksen; and his children, Warren, Dave, Gary Lee, Kim and Brandilyn. He is also survived by his 15 grandchildren, one great grandchild and many nieces and nephews.
