He was predeceased by: his mother Susan Blake. He is survived by: his father Steven Blake; his stepmother Koelyn Blake; and his siblings, Cindy Gottfredson (Eric), William Blake (Shantell), Amy Murri (Marc), Tami Bauer (Chris) and Lisa Brown (John). He is also survived by 26 nieces and nephews, and three great-nieces and nephews.
