He was predeceased by: his son David Polk Jr.; and his brother Charles Anthony Polk. He is survived by: his wife Lavera Lemons Polk of Yoakum; his children, Natalie Nichols of Cuero, Sharon Polk of San Antonio, Raymond Polk of Victoria, Richard Nichols of Cuero, Charles Polk of Killeen, Alexander Polk of Utah and Michael Polk of Yoakum; and his siblings, Margaret Thomas (Walter) of Cuero, Annie Henderson, Chester Polk Jr. (Mildred) of New Jersey and Johnny Polk of Yoakum. He is also survived by 13 grandchildren and 6 great-grandchildren.
