He was predeceased by: his parents, John Dornak and Rosie Shimek Dornak; his son Curtis Wayne Dornak; and his siblings, Jerome (Dorothy Dornak), Victor (Mary Ann Dornak), Bobby Dornak and Florence (Leo Paladino).
He is survived by: his wife Margie; his children, John Paul of San Felipe, Kevin (Sandy) of Frydek, Amy Saxton (Ben) of Webster, Kriss Parker (Pam) of Columbus and Cheryl Urbanovsky (Tommy) of Wharton; his sisters, Bernice Buss of El Campo and Norma Treybig (Tommy) of Victoria; his grandchildren, Kori Ammerman (Clay), Mackenzie Frankovic (Hunter), Kirk Parker, Kendall Dornak, Dillion Dornak, Tristan Saxton and Brodie Saxton; and his great grandchildren, Cru Alvin Ammerman, Harrison George Frankovic and Ava Grace Ammerman.
