He was predeceased by: his father Oldrich Hlavinka; and his brothers, Frank, Edward and Arnold Hlavinka. He is survived by: his wife Annette Hlavinka; his children, Jeffrey Hlavinka (Rita) of Wharton, Gregory Hlavinka of Lane City and Cindy Kortegast (Eric) of Fulshear; his sisters, Carolyn Hlavinka of Wharton and Pattye Mellen of Galveston; his grandchildren, Justin Hlavinka (Skye), Brittany Acklin (Shawn), Elyse Schubert (Jared), Eli Hlavinka, Cameron Kortegast and Chris Kortegast; and his great grandchildren, Madison Acklin and Everly Hlavinka.
In lieu of flowers, donations may be made to American Diabetes Association or American Cancer Society.
