He was predeceased by: his parents, Virginia Marie Vogel Cook and Donald Joseph Cook Sr.; and his brothers, Michael Anthony Cook and Ronald Joseph Cook. He is survived by: his wife Melodie H. Cook; his children, Dennis Wayne Boudreau (Amanda), Nicholas Blane Cook and Nicole Boudreau Russell; his grandchildren, Beau Nicholas Boudreau, Adalie Grace Boudreau, Cobra Lee Russell, Grace-Leigh Russell and Greyson Thomas Boudreau; his siblings, Donna Cook Swall and Donald Joseph Cook, Jr. (Debra); and his father-in-law Rudy A. Herrera, Sr.. He is also survived by many brothers-in-law, sisters-in-law, nieces, nephews and great-nieces and nephews.
