He was predeceased by: his child Troy; and his sister Joyce (Bob Luke). He is survived by: his wife Carolyn; his grandchildren, Taylor, Marshall, Reagan and Riley; his sister Carole (Mike Heckman); his nephew Glenn; and his niece Meredith.

In lieu of flowers the family requests donations to be made to Autism Speaks, 1060 State Rd., 2nd Floor/Data Resource, Princeton, New Jersey, 08540, in memory of Jack Richard Johnson.
