Worth, Texas and Kirk Julius Dornak (Madelyn Alldredge) of Ft. Worth, Texas.; his uncle Jim Gann; his brothers, Allen Dornak (Donna Dornak) of Dallas, Texas, Darrell Dornak (Mona Dornak) of Victoria, Texas and Jeff Dornak of Austin, Texas.; his nephews, Matt Dornak, Mitchell Dornak, Brad Dornak, Luke Dornak, Sam Wasmuth, Hank Wasmuth and Mac Wasmuth; and his niece LeeAnn Dornak. He is also survived by his family and friends.
