He was predeceased by: his parents, Edwin W. Slates and Laura M. Slates (Mapel); his daughter Pam King (Danny King); and his siblings, Jerry Slates and Norma Jean Prah.

He is survived by: his wife Shirley R. Slates; his children, Deborah Childers (Greg), David Slates, Laura Trammell, Jeff Slates (Karol), Paula Matthews and Barbara Jennings (Ned); and his sister Barbara Polen. He is also survived by sixteen grandchildren, twenty-two great-grandchildren, and by one great-great grandchild.
