He was predeceased by: his grandparents, Rick, Wanda Stewart, Judy Mayes and Liz Mayes. He is survived by: his mother Melinda Perry; his stepfather John Perry; his sister Lilly Perry; his father Chad Mayes; and his grandparents, Poppy Jack, Mimi Patricia Perry, Kevin Ogletree and Royce Mayes. He is also survived by numerous loving aunts, uncles, cousins, and friends.
In lieu of flowers, memorial donations may be given to the BDSRA Foundation (Batten Disease Support and Research Association), Make-A-Wish Foundation, or Bridges Training Foundation in Midlothian, Texas.
