He is survived by: his daughters, Cathryn Leigh and Kimberly Marie; his wife Susan Lynn Mellina; his son Dennis Ray; his son-in-law Marcus Henderson; his grandchildren, Marcus Jr. and Morgan; and his brothers, Gaylon Lee and Jerry Don Hayslip of Roosevelt. He is also survived by numerous nieces and nephews.
