He was predeceased by: his parents, Peter Anthony Fazio and Florence Allocco Fazio; and his brother Peter Fazio. He is survived by: his wife Lorise Delores White; his sons, Mario Fazio (Amanda) and Roman Fazio; his siblings, Mary Ann Rafac (Paul), David (Carma) and Michael (Karen); his siblings-in-law, Diane and Richard E. Bell; and also, Letha Jeanpierre (Andrew), LeMira McCoy (Aaron), Jamelle White and Jerilyne Mills (Darrell). He is also survived by numerous nieces and nephews.
