He was predeceased by: his parents, Jay Smith and Judy Johnson. He is survived by: his wife Dorothy; his children, Adam Jay Smith (Kelly), Andrew "Andy" Joe Smith and Serita Hope Smith (Keith); his grandson Brady Christopher Smith; and his brother Jerry.
In lieu of flowers, the family asks you to consider a donation to The American Cancer Society or Alzheimer's Association to help others as Chris would want you to do.
