He was predeceased by: his parents, William Bryan and Goldie Faye Forrest (Vance); his siblings, Howard (Nellie), Donald (Helen), Robert (Joan), Warren Forrest (Gail), Alice Osborne (Bill), Norma Dieske (Ralph) and Dorothy Ward (Dick); and his grandson Jon Ryan Forrest.
He is survived by: his wife Gertrude Marie Bolen; his sons, Ryan (Lou Ellen), Gordon and Douglas Forrest (Nancy); and his grandchildren, Amanda Slabach (Nathan), Paul Michael Forrest (Lydia), Timothy Forrest (Liza), Abigail Srader (Shawn), Emily Boncek (Steven), Kathryn Peters (Ryan), Adam Forrest (Jana), Suzanne Penfold (Casey) and Patrick Forrest (Molly). He is also survived by 20 great grandchildren.
