He was predeceased by: his father Dan Slater. He is survived by: his mother Betty Slater; his children, Alexis Slater, Allison Slater and Justin Slater; his grandchildren, Dean and Sam; and his brother Steve Slater. He is also survived by his nieces and nephews.

Jerry's family requests that in lieu of flowers, those donations instead be made to St Judes Children's Hospital or Bastrop County Animal Shelter.
