She was predeceased by: her parents, Bob Lezak and Madine Lezak (Krancher); her sister Beverly Koym; and her parents-in-law, Edwin Haidusek and Frances Haidusek.
She is survived by: her husband Rick; her sons, Clayton Haidusek (Kristin) and Caleb Haidusek (Ashley); her brother Tim Lezak; her nephew Tyler Lezak; and her grandchildren, Kallie Haidusek, Camden, Connor, Creed, Liam and Carter Haidusek.
