He is survived by: his former wife Tammy Hollar; his daughter Nicole Conrad (Chase); his grandson Jackson Flores; his mother Rose Marie Hollar; his siblings, Gayle Hollar and John Hollar (Robin); his nephew Christopher Hollar (Shannon); his great-nieces and great-nephews, Knox, Everly and Hadley; his nephew Nicholas Hollar (Ashley); his nephews, Evan Hollar (Emily), Bently and Lucas; his niece Mackenzie Durbin (Ryan); his great-nieces and great-nephews, Sadie and Aria; his niece Avery Hollar; and also Jaxson.

In lieu of flowers, the family requests donations be made to the St. Jude Children's Research Hospital in his memory.
