He was predeceased by: his parents, Hubert "Big Tiny" Hazel and Katherine "Pithie" Hazel; and his siblings, Clarence Perkins, Daniel "Boon" McNeil, Freddrick Lee Holt, Billy Hazel and Althea Marie "Re-Re" Hazel-Baines.
He is survived by: his wife Martha Hazel; his children, Crystal Falo (Barbara), Desiree Hazel (Shaun), Desmona "Mona" Hazel (Ugene), Destiny Mason (Robert), Sommer Hazel, Lester "Jerome" Millett, Forrest Blackwell and Austin "AJ" Hazel; the mothers of children, Wendy Carpenter, Karen Hodges-Catlin, Gloria Rangel and Tonette Temple; and his siblings, Hubert "Tiny" Hazel, Larry Hazel and Dorothy Jean "Gina" Hammond. He is also survived by numerous nieces, nephews, and cousins.
