He was predeceased by: his aunt Julianna Allen; his grandfather Dwayne Cook; his great-grandparent Billy Cook; and his great-great-grandparents, Cecil and Sunshine Spicer.
He is survived by: his mother Megan Allen Cook; his grandparents, Steve Allen Jr., Dr. Christy Shank, Jolee Arancibia, Robert Odell, Shelly Stonich and Oscar Navarro; his great-grandparents, Steve, Julia Allen, Bert, Wendy Stonich, Linda Barfknecht, Debra, Mark Printup and Paul Walschmidt; his aunt Lauren Tilton (Nathan Herbert); his uncle Dustin Cook; his great-aunt Ashley Tibbetts (Josh); his cousins, Leah, Mason Herbert, Addison and Britten Tibbetts; and also, Bacon, Shank, Erickson, Owens, Campbell and Greenfield.
