He was predeceased by: his parents, Herbert Jack and Jan Jack. He is survived by: his son Christopher Jack; his sister Kathy Jack (Susie); his fiancée Debbie Leopard; and his cousins, Karen Anderson and Cheri Allen.

In lieu of flowers the family asks that donations be made to any of the following: The Wounded Warrior Project, The Gary Sinise Foundation, The Remember The Alamo Foundation.
