He was predeceased by: his parents, John W. Cook and Linda Jane Fletcher; his grandparents, John D., Maude Cook, Travis and Miriam Hunt; his brothers, Shawn Allen Cook and Phillip Nathan Cook; and his nephew Dustin James Cook.
He is survived by: his son Ryan Travis Cook (Shannon); his grandsons, Travis Grant Cook, Nolan Hunt Cook and Benjamin Rich Cook; his stepdaughter Cassie M. Cantu; his brother John Matthew Cook (Tara); his nephew John Dylan Cook; his niece Elizabeth Cook; his former wife Lynn Cook; and his stepparents, Ben Fletcher and Jessica Cook. He is also survived by numerous extended family and friends.
